Medina Memorial Hospital's nationally accredited Comprehensive Medical Rehabilitation program is the only facility in the eight counties of western New York accredited by the prestigious Commission on Accreditation for Rehabilitation Facilities (CARF). Voluntary CARF accreditation demonstrates that an organization has made a commitment to meet nationally recognized standards for quality and to continually seek excellence in its services and programs.

Rehabilitation is a process of learning, relearning and practicing skills in order to be as self-sufficient as possible. The goal of our highly qualified staff is to maximize independent functioning for patients after illness or injury, thereby greatly improving quality of life.
Complete Services Our Comprehensive Medical Rehabilitation Unit provides a complete program to meet specific rehabilitation needs required by different illnesses and injuries. Both acute and sub-acute rehabilitation services are provided, where services are stepped up or down depending upon specific needs.
 Additionally, the Comprehensive Medical Rehabilitation provides an in-depth education program for family members and caregivers so that the patient is well prepared upon discharge. The success of the program is measured by utilizing a national data bank system (Uniform Data System). The data demonstrates improvement from admission to discharge and follow-up. Our specialized team of healthcare professionals work together, using individual expertise in partnership with each other and patients to help achieve maximum functioning at home, work, school and in the community.
 Additional Services Diagnostic Testing – any diagnostic testing such as lab tests or x-rays can be performed on site, without the need to travel to another facility. IV therapy –Intravenous feeding and antibiotic therapy needs can easily be accommodated. Outpatient Services – Outpatient services are provided at two locations: Lake Plains Physical and Occupational Therapy in Medina (for Physical Therapy, Speech Therapy and Occupational Therapy), and at Arnold Gregory Ambulatory Services in Albion (Physical Therapy). Home Care Services – for at-home health services Free Parking – Free parking is available immediately adjacent to Medina Memorial. Private Rooms – If available, private rooms can be provided for an additional $10.00 per day.
Program Admission Candidates for admission to all levels of care will receive an onsite pre-admission evaluation. The patient's medical, physical, cognitive, behavioral, and social status will be evaluated for the patient's appropriateness for improvement. The patient's financial status and need for resource assistance will be assessed. A referral will be made to the appropriate agency as indicated. The family, referring physician and external case manager will be consulted as needed.
Admission Criteria: Medical Stability Rehabilitation Nursing Care Required 24 Hours/Day Able to tolerate 3 Hours of Therapy/Day, for a minimum of 5 Days/Week, up to 7 days a week. Reasonable Expectation for Improvement Patient Is Cooperative and Motivated
Admission Process: Referrals Are Accepted From Any Source. Pre-Admission Assessments Are Provided Within 24 Hours And Typically The Same Day. Admissions may be accepted 7 days / week.
