The Transforming the Care at the Bedside initiative, is a national program of the Robert Wood Johnson Foundation, Institute for Healthcare Improvement, and American Organization of Nurse Executives.

You Are At The Center Of Your Care
A stay in the hospital is often a part of the healing process. Medina Memorial staff do everything possible to make your hospital stay a comfortable and caring part of your healing process. Our highly trained care team can devote individualized attention to you and your family. You'll be cared for by a team of health-care professionals in a hospital whose staff is dedicated to providing you with excellent care and helping you be as comfortable as possible while you are here.
Our recently renovated Medical/Surgical and Obstetrical units have been designed to provide a comfortable, homelike atmosphere, with room temperatures that can be individually adjusted by the patient. Attractive, comfortable lounge/visiting areas are available to visitors and patients.
Treatment at Medina Memorial is a cooperative effort between patient and staff. With our participative patient care system, each patient is at the center of their health care.
Patients, with their expertise and understanding of their own needs, are the primary member of the health care team, and are fully involved at all stages of their care. We deal quickly with your questions, problems and concerns, and keep you fully informed about options and choices.
To maximize communication and coordination of care, patients are assigned a Primary Care Nurse, a concept that brings the registered nurse back to the bedside to provide clinical care. This means that the same nurse will assess your needs, plan your care, provide care, and give medications throughout your day. Care-giving is family focused, and if desired by the patient, family involvement in care is allowed and encouraged.
Care Management Care Managers help assess patient care needs that are present on admission or that may surface during hospitalization. Care Managers in the hospital are Registered Nurses. Your Care Manager will help facilitate your care with the other members of the health care team. Care Managers are available to assist patients with any questions or concerns during and after their hospitalization. Your Care Manager can help you plan for visiting nurse services, nursing home placement, home health care, special equipment, or other follow-up services.
