You can save time by pre-registering the evening before your scheduled appointment. This service is available between 5PM and 9PM, Monday through Friday, by calling (585) 798-8092.

Is everyone required to register?
Yes - all patients receiving services at Medina Memorial Hospital are required to register in the Patient Registration Department.
Where is the Patient Registration Department located?
It is located on the ground floor, adjacent to the main lobby. If you have difficulty finding it, someone at the Information Desk will be able to point you in the right direction.
What should I bring with me?
You should have a prescription from your doctor or other healthcare provider, along with billing information and insurance card.
When do I pay for my services?
Insurance copays are requested before services are rendered, except for in emergency situations. Copays and referral/preauthorization requirements vary by insurance policy. If you have questions, we urge you to contact your insurance provider before beginning the registration process.
Patient Satisfaction Surveys
Your comments and suggestions help to shape the way we deliver services to the community. We give every patient a Patient Satisfaction Survey, and are greatly appreciative when people take the time to fill it out and return it to us. You can either leave it with us or send it back to us in the pre-paid envelope when you get home.
